Drinking Poison: Pakistan's Hidden Public Health Emergency

Every day in Pakistan, children are admitted to hospital with diarrhoea, typhoid, hepatitis A, and cholera. Most of these illnesses trace back to a single source: contaminated drinking water. As a physician who has spent fifteen years treating these patients, I can tell you the scale of this crisis is deeply underreported.

The Statistics We Rarely Talk About

According to the Pakistan Council of Research in Water Resources, over 80% of diseases in Pakistan are water-related. Diarrhoeal diseases alone are responsible for the deaths of an estimated 53,000 Pakistani children under the age of five annually. Typhoid fever, caused by contaminated water, is endemic across most of urban Pakistan.

In my clinic in Karachi, I regularly see children who have been drinking water contaminated with E. coli, arsenic, or industrial chemicals. Their families often have no idea. The water looks clear. It may have been "filtered." But filtration at the household level rarely addresses heavy metals or pharmaceutical contaminants.

"A mother came to me with her four-year-old who had typhoid for the third time. She was heartbroken and confused. She said: "I boil the water, doctor. I do everything right." The problem was not her practice — it was her source."

How Water Becomes Contaminated

🌾Agricultural runoff carrying pesticides and fertilisers into groundwater

The Tanker Question

Private water tankers have become a primary drinking water source for millions of Pakistani households. From a public health perspective, this is deeply concerning — because there is almost no regulation of tanker hygiene, water source quality, or delivery standards.

I welcome platforms that introduce driver verification and accountability into the tanker sector. But I would urge them to go further: water quality testing at source, hygienic tank maintenance standards, and customer reporting mechanisms for suspected contamination. Safe delivery is not just about getting water to the door — it is about ensuring what arrives is actually safe to drink.

What Families Can Do Now

Until systemic improvements arrive, I advise patients to boil water before consumption, use certified filtration systems, and — critically — know their tanker source. Technology that makes tanker sources transparent and accountable is not a luxury. It is a public health tool.
